Presenter:
Tabitha Johnson
Ms. Johnson is the Founder and CEO of Dementia Education & Consulting, Ltd., providing the pathway for individuals to become a Certified Dementia Practitioner through the National Council of Certified Dementia Practitioners (NCCDP.) Johnson is also the creator of the Simulated Dementia Journey; an interactive program demonstrating both the physical and psychological issues that are endured when living with Dementia.
She worked in the long-term care field for over 9 years with experience as a Certified Nursing Assistant (CNA), Medical Records Assistant, and Social Services Director. She served as the president for the Georgia Society of Social Service Professionals (GSSSP), Georgia Health Care Association (GHCA) board as the Social Worker Liaison and as the President of the Northwest Council for GSSSP.
Tabitha is a Certified Dementia Practitioner (CDP), Certified Alzheimer’s Disease & Dementia Care Trainer (CADDCT), and a Certified Eden Associate (CEA), Virtual Dementia Tour Facilitator (VDT-F), Family Support Group Facilitator (FSGF) for the National Alliance of Mental Illness (NAMI), Certified Dementia Support Group Facilitator (CDSGF), and Certified Montessori Dementia Care Professional (CMDCP). She holds a Master’s degree in Healthcare Administration (MBSHA) with a concentration in Gerontology.
Ms. Johnson and her husband, Tobby, have recently opened a restaurant in Historical Downtown Dalton, Georgia called Sumpthin Southern Kitchen. They serve fresh, cooked daily, southern food. The Johnson’s are working closely with Purple Table Reservations to transform their dining area into a Dementia Friendly Restaurant. Special accommodations are being set also for those who have Autism, PTSD, hearing or vision impairment, or other physical and cognitive conditions.

Course materials included. After completion of the seminar, the application for certification must
be submitted within 30 days. The group discount rate for certification
is $35.00 and must be paid to the NCCDP.
CONTINUING EDUCATION
The
Kentucky Association of Health Care Facilities is an approved provider
of continuing education by the Kentucky Board of Nursing, #5-0002. To
receive the contact hours, you must complete the entire program.
Certificates may be printed once the program evaluation is submitted. Disclaimer

CANCELLATION POLICY
Cancellation
received in writing no later than seven (7) workdays prior to a seminar
will receive a full refund. Cancellations received in writing inside of
seven (7) workdays will receive one-half refund. Any participants who
don't attend a program will receive no refund unless prior notice in
writing is given to the association office as outlined in this policy.
Participants may appeal refund determinations by requesting
reconsideration by the Executive Committee. Such appeals must be made in
writing within 30 days after the seminar or convention is held. Please
