Eleisha
Wilkes is a Registered Nurse and AANAC certified MDS consultant with
over fifteen years of experience in long term care. She has served as a
Director of Nursing, Case Manager and Resident Assessment Coordinator
with extensive responsibility and success in driving QA in the areas of
person-centered assessment and care planning through effective systems,
staff development, and interdisciplinary team collaboration. Eleisha
specializes in MDS validation audits, Medicare and Medicaid
reimbursement compliance, competency-based staff education programs, and
works with clients extensively to facilitate SNF/NF survey readiness
including Phase 1-3 Requirements of Participation compliance. She has
presented extensively on the Patient Driven Payment Model and nursing
facility readiness under the new reimbursement system. She now serves as
an RN Clinical Consultant with Proactive Medical Review &
Consulting, LLC.
Kristen Walden, RN, MSN, RAC-CT, Clinical Consultant
Kristen
Walden brings twenty-five years of experience as a corporate clinical
and reimbursement nurse, DON, and QA Nurse with expertise in driving
nursing compliance, implementing software solutions, MDS/RAC skill
development, and systems improvement for Five Star, clinical quality and
reimbursement. She specializes in teaching clinical problem solving
and advancing quality operations for consistent outcomes.

Continuing Education
The
Kentucky Association of Health Care Facilities is an approved provider
of continuing education by the Kentucky Board of Nursing, #5-0002. To
receive the 1.25 contact hours, you must complete the entire program.
Certificates may be printed once the program evaluation is submitted. Disclaimer

Cancellation Policy
Cancellations must be made in writing and emailed to [email protected] or faxed to 502-425-3431. Cancellations
by phone will not be accepted. Cancellations received no later than February 14 will receive a full refund. Cancellations received between February 14 to February 20 will receive a refund of 1/2. Registrants who
do not attend or who do not complete the full seminar will not receive a
refund unless prior written notice is given to the KAHCF office.
